THE POSITION:
The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking experienced Clinical Social Workers to join our team within Western Connecticut Mental Health Network (WCMHN).

Schedules: Both positions are part-time (32 Hour)
- PCN 141806: Monday - Thursday | (8:00 A.M. - 4:30 P.M.)
- PCN 141807: Tuesday - Friday | (8:00 A.M. - 4:30 P.M.)
- Unit: Jail Diversion Unit under the Community Reentry Program (CORP)

DISCOVER THE OPPORTUNITY TO:
- Serve as a member of a multi-disciplinary clinical team under the Community Reentry Program (CORP) working collaboratively with community providers, other state operated service providers, Court Support Services Division, local and statewide hospitals, and liaison between agencies and Western CT Mental Health- Waterbury Area.
- Travel to various Department of Correction (DOC) facilities to co-lead psycho-educational groups, engage with clients, facilitate discharge planning from the correctional facility to WCMHN-Waterbury Area, and provide case management services to clients upon release and facilitate engagement.
- Conduct intake assessments to determine level of care for clients who will be served by WCMHN-Waterbury Area.
- Develop thorough clinical interviews with clients and families and treatment plans including measurable treatment objectives
- Provides clinical treatment services in the form of triage, crisis intervention, individual, group and/or family therapy, screening and planning.

The network operates in Waterbury, Danbury, and Torrington, with over 200 dedicated staff offering comprehensive services for individuals with long-term mental illnesses.
