The ideal candidate will have prior experience in service coordination/case management or the field of human services. Bilingual in Spanish or ASL is a plus!
Traditional Service Coordinator
Schedule: Monday - Friday, 8:30am-4:30pm, hybrid position
Salary: Starting at $21.00-$23.00
POSITION SUMMARY:
To provide exemplary customer service to individuals, families, and teams served. Provide coordination of services, connection to resources, assistance and advocacy to meet the needs of individuals served. Proactively meet service coordination (SC) compliance measures and ensure quality integrity per He-M. 503 regulations, agency best practices with a person-centered focus. Seek creative solutions in a timely fashion.
ESSENTIAL JOB FUNCTIONS:
- Advocacy role: Advocate for the needs and wants of the individual to include promoting and protecting individual rights; report any human rights violations or service concerns.
- Facilitate the service agreement process, quarterly team meetings and other meetings as necessary
- Assist with referrals for consultation and treatment as needed.
- Facilitate Service Authorization process for individuals to access support services dependent upon their needs (i.e. residential /vocational, medical, dental, psychological services, adaptive equipment, community resources, etc.).
- Facilitate communication and maintain effective professional relationships with all members of the team.
- Maintain a working knowledge and adhere to all applicable state and federal program regulations.
- Identify and advocate needed funding for services – enhancements, crisis, etc.
- Access and log assistance to individual’s funds for clients who have specific needs which they cannot afford on their own.
- Use and maintain the Monthly Compliance Checklist to track and document core requirements per He-M 503.
- Complete required documentation and reporting using department tools and metrics
- Complete monthly Activity Exception Report from relational database to ensure quality wor
- Follow the Guidelines for 503 that reflect the state mandated requirements for individuals under the He-M503 regulations.
- Be able to identify clients’ needs based on Caseload Management Tool
- Effectively communicate with Supervisor, team, and stakeholders
- Respond appropriately to feedback to grow, maintain, or improve performance accordingly.
- Ensure through facilitation and advocacy, quality delivery of services.
DUTIES AND RESPONSIBILITIES:
- Monitor services for safety, quality, and satisfaction

- Utilize funded community based organizations and resources to reduce costs and bolster client supports. Utilize community resources as needed to provide a continuum of care
- Work collaboratively with other staff/departments and programs – be responsive and respectful of agency staff across departments
- Never hold up the work of others; communication is key
- Attend agency and departmental staff meetings
- Participate in internal and community committees/groups as assigned
- Contribute to the development of healthy team relationships and departmental atmosphere
- Emergency on-call rotation
- Responsible for promoting health & safety within organization as defined by Safety Committee/Company policy
- Promote & work towards meeting agency goals and objectives as defined in the strategic plan
- Compliance with all Gateways Community Services policies and procedures
- Complete all necessary required trainings and continue to attend training
- Perform other work-related duties as assigned
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
- Bachelor’s Degree preferred or 2-3 years related experience
- Prior casework or related experience with ID/DD population and their families
- Strong belief in provision of community services for population served
- Ability to function as a member of a team and be a team builder
- Technical competence (Proficient in MS Office applications; word processing and spread sheet (Word/Excel) - Relational data base knowledge a plus
- Ability to work effectively with individuals with developmental disabilities and ability to advocate and help them advocate on their behalf
- Effective interpersonal and communication skills
- Good organizational/time management skills
- Knowledge of Gateways Community Services, Greater Nashua community and available resources
- Valid drivers’ license and applicable vehicle insurance for essential transportation and travel
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
- Occasional standing
- Frequent sitting, minimum of 5 hours per day
- Occasional walking, bending, and twisting of spine
- Light lifting and carrying, less than 30 lbs.
- Visual acuity-worker is required to have close visual acuity to perform job functions
