Discrete Targeted Case Manager

Qualified candidates who are offered a position by September 1st will receive $500 when they start and another $500 when they have completed their introductory period.
Key Responsibilities
The Adult Services Department of SMHC is looking for an energetic, dedicated, experienced individual to join our Portsmouth team. This position requires connecting clients with community resources, coordinating services, and providing advocacy and assertive outreach to adults with mental illness. The ideal applicant must possess a Bachelor's degree in Social Work, Psychology, Human Services or the equivalent and preferably have one year of experience working in the field. 
  1. The Case Manager will be able to accurately describe targeted case management services to clients, families/guardians, and community providers.
  1. The Case Manager will accurately assess the case management needs of the client while completing the TCM assessment in order to provide the most effective services to meet the client’s needs. These needs are often diverse and will vary from one client to the next. 
  1. The Case Manager will provide direct and/or indirect case management services to the clients monthly or more frequently as needed, with at least one face-to-face meeting with the client every 90 days. Services are delivered in the agency, home, or community settings. 
  1. The Case Manager will develop client driven TCM goals and objectives that are consistent with the TCM assessment. The Case Manager will complete the TCM Care Plan and monitor the client’s progress on a monthly basis.
  1. The Case Manager will develop and maintain a tracking system to accurately identify the TCM needs of the clients being served and provide a copy to his/her Supervisor on a monthly basis.
  1. The Case Manager will maintain an organized system of resource materials to provide information on resources, including but not limited to: advocacy groups, disability rights organizations, recreational programs, financial assistance agencies, rent/fuel assistance programs, homeless shelters, food banks/pantries, soup kitchens, legal assistance, transportation, health/dental centers, parenting support programs/groups, and other agencies as needed.
  1. The Case Manager is a part of the client’s treatment team and is responsible for relaying information through the appropriate channels. This may include attendance at team meetings or consults with other providers as needed. 
  1. The Case Manager is required to attend supervision, team meetings, and in-services on a regular basis, as designated by the Supervisor.  
  1. The Case Manager must be aware of and adhere to all SMHC Policies and Procedures. 
  1. The Case Manager will be aware of and adhere to state and federal mandates regarding service provisions.
  1. The Case Manager is required to keep regular documentation of all activities. 
  1. The Case Manager will meet the clinical performance standards as developed by the agency.
  1. The Case Manager will need to work a flexible, nontraditional schedule to ensure the continuity of care and optimal support services. Regular job attendance is required.
  1. The Case Manager will complete other duties as assigned.
Other Requirements 
  • Must also demonstrate excellent organizational and communication skills, both verbally and in writing.
  • Must hold a valid driver’s license and be able to demonstrate a "clean" driving record.
  • CPR/First Aid training is recommended.
