Physical Therapist

 

JOB DESCRIPTION

 

Job Title: Physical Therapist (PT)

Job Class:  Exempt

Date: 4/1/2025

Reports To:  PT Supervisor

 

ESSENTIAL JOB FUNCTIONS 

  • Work under regular direction according to the agency and Birth to Three mission and philosophy 
  • Provides evaluation, assessment, intervention and service coordination, in a child’s natural learning environment, as part of a multidisciplinary team for children with developmental delays. 
  • Collaborate with the family and a multi-disciplinary team to develop goals and outcomes for children receiving Birth to Three support
  • Coach family members and caregivers on different intervention strategies to facilitate active involvement and carry-over in community settings 
  • Maintain an accurate record of services provided and submit all documentation including but not limited to session notes, evaluation reports, service coordination and progress reports in a timely manner 
  • Maintain proper documentation to comply with all state regulations

 

REQUIREMENTS

  • Must hold a Master’s Degree in Physical Therapy from an accredited school 
  • Must be licensed in the state of Connecticut 

 

PHYSICAL REQUIREMENTS

  • Hand dexterity ability (ability to operate mobile device, telephone, computer) 
  • Ability to travel to multiple locations
